The role
The Team Facilitator Support Coordination is responsible for providing quality services to HelpingMinds clients as well as coaching and mentoring the Support Coordination Team ensuring that the organisations values are upheld and championing changes required organisationally to drive stronger performance.
Accountabilities include but are not limited to:
- Coaching and mentoring to the team in their duties ensuring that policies, procedures and documentation requirements are followed to enhance quality service delivery.
- Taking ownership of team based key performance indicators
- Provide advice and feedback to the team on individual service delivery performance and developmental needs via case review meetings.
- Facilitate the onboarding of clients through the Individualised Services Intake process and assess support needs of participants
- Act as a role model and advocate for best outcomes for people with a psychosocial disability
- Ensure services are provided in ways that reflects the underpinning values and principles for supporting people with a psychosocial disability.
- Undertake risk assessments and management of workplace practices and client’s living environment to mitigate potential risk.
- Develop, implement, monitor and review client’s individual plans which include behaviour management and complex health plans in consultation with key stake holders.
- Provide accurate and timely reports
- Onboarding all referrals with responsibility for completion of all Individualised Service intake processes.
- Research and identify providers and other relevant stakeholders and build strong working relationships
- Assist participants in successfully implementing their NDIS plan;
- Complete support coordination tasks within agreed timeline in partnership with Psychosocial Recovery Coachers, Support Coordinators and Specialist Support Coordinators
The position will report to the Executive Individualised Services and is offered on a Permanent full-time basis. Whilst the position is primarily based out of our Perth office there are working from home options and some travel within the Perth Metro area required.
About You
The successful candidate will have:
Minimum qualifications:
- A tertiary qualification in Mental Health or Disability Services OR Diploma/Cert IV Mental Health and minimum of 3 + years experience working in a similar position
- Current Drivers License, Working With Children Check, First Aid Certificate
- NDIS Worker Screening Clearance and Quality, safety & You – NDIS Worker Orientation Module Certificate
- In line with HelpingMinds policy, WA Government Health Directions and Public Health Orders, the successful candidate will need to be fully vaccinated against COVID-19 by 31 December 2021 and as such will need to provide evidence of their COVID-19 Vaccination status prior to commencement.
Preferred experience:
- Demonstrated skills and experience working in Mental Health Support including working with psychosocial clients on NDIS plan implementation
- Demonstrated excellent communication skills including report writing skills
- Capacity to build rapport with clients and other staff members is crucial
- Demonstrated ability to communicate with a diverse range of people
- Demonstrated ability to consult, liaise and maintain close working relationships with other service providers
- Demonstrated ability to be self-motivated and function autonomously while working effectively toward team goals
- Demonstrated capacity to engage with and empower people to achieve their identified goals within a structured individual recovery plan
- Ability to work collaboratively with a team, clients and other agencies
- Strong leadership and management skills
- Strong administrative, organisational and planning skills with high level attention to detail
- Well-developed IT skills, with experience using Microsoft products and databases
- Ability to work collaboratively with a team, clients and other agencies
